My GP has said I am eligible – what do I do now?

Once your doctor has determined that you met the eligibility requirements you need to:

form

  1. Give your Doctor the clinic details as stated above so that they can organise a Team Care Arrangement with us. Your Doctor will give you a referral form along with any necessary health information that the Chiropractor will need. The form your Doctor gives you will look similar to the picture shown.
  2. Once you have that from, bring it and any other health information to your appointment. We recommend that you ask for a copy and bring the forms with you. Although your Doctor is likely to offer to email those forms through directly to the clinic, they often get delayed and do not make it in time for your appointment. Lakeside Laser will need the form and the details it provides in order to process the bulk billing. You will also need to bring in your Medicare card.
  3. After your appointment we will then process your payment through to Medicare directly for the cost with no gap payment for you to pay at all.
  4. When you have completed your bulk bulled appointments for low level laser therapy, your chiropractor will write a medical progress report and send it to your GP updating them on your response to treatment and the benefits obtained.
